Stale context is worse than no context -- an AI with nothing to go on says it does not know, while an AI with a six-week-old plan produces a confident, fluent, wrong answer. The record is a list of decisions, risks, questions and commitments in plain language, each one openable -- not an opaque store of embeddings. Any record can be corrected in place, from the app or from an AI tool over MCP: titles, rationales and statuses can be changed, context captured against the wrong workspace can be reassigned, questions can be resolved, commitments closed, and outdated goals extended, finished or archived. A correction applies for everyone, not only for one chat session. Attribution is the exception: who made a decision stays fixed, and a revision belongs in the rationale or in a follow-up decision. ### Limits In Parallel cannot capture a decision that was never said out loud.
